1. Our commitment and this policy’s scope
Rae’s Playze Adult Day Center (“Rae’s Playze,” “we,” “us,” or “our”) respects the privacy and dignity of members, families, applicants, staff and website visitors. This policy explains how information is handled through raesplayze.com and related website communications.

Member health and service records are also protected by applicable federal and North Carolina confidentiality requirements. Our separate HIPAA Notice of Privacy Practices explains how protected health information may be used and disclosed and how applicable health-information rights may be exercised. This website policy does not replace that notice, authorize disclosure of confidential member information or limit rights under law.

2. Information provided to us
When you contact us, request a tour, ask about services or complete a form, we receive the information you submit. This may include your name, telephone number, email address, relationship to a prospective member and message. Enrollment, referral and employment materials may request additional information relevant to their purpose. The information collected depends on the form or service you choose.

Provide only information needed for your request. Do not enter diagnoses, medical records, Social Security numbers, insurance identifiers, financial account details or other sensitive information in a general contact form or ordinary email. Call us for instructions on an appropriate submission method. Do not submit another person’s confidential information unless you are authorized to do so.

3. Website technology, cookies and outside content
Website hosting and related tools may process technical information such as IP address, browser and device information, pages requested, access times and security logs. Cookies and similar technologies may support site functions, remember preferences or enable third-party content.

Our website includes Google Maps and may offer other third-party content. Loading or interacting with outside content may send technical information to that provider, which handles information under its own privacy policy. Requesting contact allows us to respond about that request. It does not by itself enroll you in marketing campaigns or authorize disclosure of member records. Where newsletters or promotional communications are offered, you may decline them or use the unsubscribe option provided. Service-related communications are governed by applicable consent requirements and your communication preferences.

5. When information may be shared
Information may be accessed by authorized personnel and service providers who assist with website operations or the purpose of your request, subject to appropriate safeguards and applicable legal requirements. When information is protected health information, applicable authorization requirements, business associate arrangements and confidentiality protections apply.

We may disclose information when legally required or otherwise legally permitted, including appropriate responses to valid legal process or required reports. Confidential member information is disclosed only with a valid authorization or another applicable legal basis. Use of this website is not an authorization to release a member’s information to relatives, advertisers or other third parties.

6. Member photographs, videos and testimonials
Identifiable member images, recordings, stories and testimonials used for public promotion require appropriate documented permission and, where applicable, a valid health-information authorization. We respect the member’s decision-making authority and verify a representative’s authority when relevant.

Declining promotional participation will not affect access to services. You may contact us to withdraw permission for future use, subject to applicable law and the terms of the authorization. Withdrawal cannot undo authorized actions already taken or guarantee removal of copies made by others. Sending a message or photograph does not automatically transfer ownership or grant permission for public use.

7. Safeguards and retention
We use administrative, technical and physical safeguards appropriate to the information handled. Internet communications carry risks, and no website or transmission can be guaranteed completely secure. Contact us for a suitable method of exchanging sensitive information.

Information is retained according to its purpose, applicable record-retention requirements and legitimate legal or operational needs. When no longer needed and eligible for disposal, it is securely disposed of or de-identified as appropriate. Some records must be retained even when deletion is requested. If an incident requires notification under applicable law, we will provide the required notification.

8. Requests, choices and complaints
You may contact us to ask about information submitted through the website, request a correction, request deletion where permitted, change communication preferences or raise a privacy concern. We may verify identity and representative authority before responding. Applicable health-record rights and their limits are described in our HIPAA Notice of Privacy Practices.

You may raise concerns without retaliation. Ask us for the member grievance procedure and assistance using it. You may also contact an appropriate licensing authority, protection and advocacy organization, payer or the U.S. Department of Health and Human Services Office for Civil Rights when applicable. You do not have to complete our internal grievance process before contacting an outside agency.

9. Children and representatives
This website is intended for general information and inquiries from adults, including parents, guardians and authorized representatives. It is not designed for children under 13 to independently submit personal information. If you believe a child has submitted information inappropriately, contact us. A representative’s access to a member’s information depends on verified authority and applicable law.

10. Changes and contact information
We may revise this policy to reflect changes in the website, our practices or legal requirements. The current policy and effective date will be posted here. Changes do not eliminate rights or authorize uses requiring separate permission.
