Community Networking Partner

Description:

Community Networking (individual or group) is a service that provides individualized day activities that support the individual’s definition of a meaningful day in an integrated community setting. A goal of this service is to be inclusive of individuals with disabilities within the community, therefore it is provided separate and apart from the home of the individual and that of the provider.

Position Requirements:

  • The CN Partner builds trust and productive relationships with people he/she supports, co-workers and others through respectful and clear verbal and written communication.
  • The CN partner uses person-centered practices, assisting individuals to make choices and plan goals, and provides services to help individuals achieve their goals.
  • The CN Partner identifies risk and behaviors that can lead to a crisis, and uses effective strategies to prevent or intervene in the crisis in collaboration with others.
  • The CN Partner helps individuals to be a part of the community through valued roles and relationships, and assists individuals with major transitions that occur in community life.
  • The CN Partner works in a professional and ethical manner, maintaining confidentiality and respecting individual and family rights
  • The CN Partner closely monitors an individual’s physical and emotional health, gathers information about the individual, and communicates observations to guide services.
  • The CN Partner respects cultural differences, and provides services and supports that fit with an individual’s preferences
  • The CN partner obtains and maintains necessary certifications and seeks opportunities to improve their skills and work practices through further education and training.
  • The CN Partner will be responsible for finding volunteer opportunities and community events for consumer to participate

Supported Employment Specialist

Description:

The Employment Specialist performs as an integral part of the management team working with adults with disabilities. The primary duties are to secure community-based employment for individuals with disabilities. This position will provide evaluation, job development, and intensive training services in the community for the individuals served and supported.

Position Requirements:

  • Accompany individuals to job interviews and assist in filling out applications and communicating qualifications
  • Accompany individuals being supported to their community-based employment site to provide on-the-job training
  • Transporting individuals to and from home, work, interviews, trainings and/or any other job related events
  • Analyze job duties and tasks and complete a thorough job task analysis
  • Assist individuals in mastering skills and meeting production standards. A job coach may be required to assist in performing an individual’s job duties to maintain necessary production as well as model work behavior.
  • Facilitate social interaction with coworkers and provide reinforcement as learning is demonstrated)
  • Conduct primary job development activities for the program
  • Maintain case records as required by licensing and funding bodies’ standards and regulations
  • Coordinate communication with parents, case manager, and other agencies serving the individuals being supported. This means that documentation must be entered into electronic record system within 24 hours of service delivery
  • Maintain good work habits regarding attendance, punctuality, appearance, and effective working relationships with co-workers, LME staff, employers, and individuals being supported
  • Provide supplemental evaluation, job development, and intensive training activities with individuals being supported according to their authorization, service plan, and work schedule

Qualified Professional

Description:

As a Qualified Professional with Rae’s Playze Adult Day Center, you will serve as the center of the interdisciplinary service team to guide the development of person-centered plans and overall programs based on the information you gain from interviewing the people we serve. You will also complete incident reports and employee performance evaluations.

Position Requirements:

  • Assisting each member we support to identify and communicate his or her requests and needs for services and supports through direct interviews
  • Supervising, integrating and coordinating person-centered plans and monitoring progress towards personal, clinical and functional outcomes
  • Initiating periodic reviews, investigations, modifications, and adjustments by soliciting the feedback of the people we support as their needs and goals change
  • Reviewing assessments completed by the people we support and other members of the interdisciplinary team to best help the people we support to meet their full potentials
  • Supervising and writing the person-centered plan based on assessments, interviews, and observations
  • Reporting to the Executive Director and working closely with colleagues to maintain communication and provide feedback, standardize procedures, expedite person-centered plan implementation and workflow and improve employee performance
  • Monitoring activities to ensure that quality assurance and/or state survey improvements are being made
  • Serving as the link between the people we serve and their families and legal guardians to gain approval for services provided computer

Additional requirements for the Qualified Professional include:

  • Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus
  • Minimum 2 years’ experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degree
  • Working knowledge of applicable regulations that apply to our service offerings
  • Valid driver’s license, auto insurance, and reliable transportation
  • Clean criminal background check
  • Supervisory/management experience and experience writing person-centered (individualized service) plans preferred

Alternative Family Living Provider

DESCRIPTION:

Rae’s Playze Adult Day Center is looking for passionate people to open their homes and their hearts to provide Alternative Family Living (AFL) Residential Support Services to individuals with special needs in Charlotte and surrounding areas. We seek hard working and caring people with a penchant for helping others. Our services go beyond taking care of individuals with Intellectual or Developmental Disabilities (I/DD). We strive to help these individuals live their lives as independently as possible and you can play a meaningful role in this process.

POSITION REQUIREMENTS:

A qualified candidate for this contractor opportunity

  • Must possess a high school diploma or its equivalent.
  • Be caring yet establish good boundaries.
  • Be willing to learn and grow.
  • Possess good communication skills (including written).
  • Possess basic computer skills.
  • Experience is preferred working with individuals with IDD/Mental Health but not required.
  • Trainings will be available including CPR/First Aid, EBPI and Medication Administration.

Day Support Provider II

DESCRIPTION:

High School Diploma or GED and four (4) years’ experience working with individuals with a developmental disability/mentally or physically challenged and personal care. Three (3) reference letters will BE needed or the names of individuals with whom a reference interview can be conducted, including at least one former employer. Must be 25 years old, clean MVR record to meet Driver/Transporter requirements.

POSITION REQUIREMENTS:

  • Performs client’s care under the direct supervision of the QA/QI Director and is responsible for performing essential care for our participants.
  • Maintains contact and accountability with the supervisor
  • Implementation of habilitation goals program will include, but not limited to, self-help (i.e. personal hygiene needs).
  • Efficient in goal documentation as evidenced by timely and accurate documentation and assessment.
  • Efficient in creating and maintaining professional relationships with guardians and family
  • Efficient in providing accurate and therapeutic feedback to Care Coordinators
  • Submission of all agency required documentation in a timely manner
  • Working one-on-one with clients, as needed.
  • Escorts and transports clients as assigned to appointments and/or activities
  • Participates in activities both on and off the site.
  • Participates in ISP (Individual Support Plan) as requested.
  • Abides by established policies relating to handling absences, tardiness, and work breaks.
  • All other duties as requested by your unit leader/supervisor.
  • Participate in required staff developmental trainings, (i.e. CPR, 1st Aide, NCI A&B, Core Training) and any other staff developmental training as requested by Rae’s Playze Adult Day & Health Center.

Respite Provider

DESCRIPTION:

  • Playing simple games with clients, going to movies, eating in restaurants and other tasks as needed.
  • Assisting clients with their eating or any needs in this area.
  • Help assist clients to stay on task, being supervised, getting materials, etc. Daily documentation of data per goals.
  • Enabling the individual to attain or maintain maximum functional level
  • Provide one on one time with individual in the community or in the consumer’s home.
  • Support a person’s regular scheduled daily activities, as needed.
  • Participate in required staff developmental trainings, (i.e. CPR, 1 st Aide, NCI A&B, Core Training) and any other developmental training as requested by Rae’s Playze Adult Day & Health Center administrative staff.
